| Details: | SUMMARY: Completed/corrected survey deficiency follow up DETAILS: A significant deficiency was cited in the survey on August 14th for not meeting setbacks. Four options for correction were provided in the cover letter. The owner/operator Mark Bolton requested and evaluation for a construction setback waiver. A formal waiver was approved 2/14/13. A letter was sent to Mr. Bolton explaining the waiver approval copy of letter provided with this contact. ACTION NEEDED: Continue to monitor coliform and nitrate testing. | ||
