Details: SUMMARY: Survey deficiency follow up DETAILS: System had not sent the 2009 CCR to the state. The system operator sent a copy of the 09 CCR to this office and I faxed he copy to the state on 1/29/13. This resolves the significant deficiency from 12/22/12 survey ACTION NEEDED: Deficiencies have been corrected.
