Details: SUMMARY: Investigation of Violations # 901650012, 901650013 DETAILS: System failed to take annual source assessment samples for SRC- AA & SRC-AB in 2012. Donna called my cell to find out why they had violations. I let her know to take the annual source assessment sample at both wells. She said that she would right away. ACTION NEEDED: Monitor & RTC violation
