| Details: | SUMMARY: Significant deficiency follow up DETAILS: Survey showed no ASR, no EA, no CCR and no O & M manual and no ERP. System worked with me and HBH, Doug Wise, to develop all of these missing documents. Package of all missing docs was submitted 1/28/13. All appears to meet criteria. ASR is being prepared and will be submitted to the state by March 31 ACTION NEEDED: Submit ASR this year and each year by 3/31 | ||
