Details: SUMMARY: Survey deficiency correction follow up DETAILS: 5 deficiencies noted during May 14, 2013 survey included: no DRC, no coliform sampling plan, no operator contract, no annual summary report and no enabling authority. Worked with system and operator for several weeks and Nate produced all missing documents on 5/20/13. Enclosed ACTION NEEDED: continue to update all of these required docs annually
