| Details: | SUMMARY: Significant deficiencies (SD) on approved corrective action plan DETAILS: Two SDs were note on the survey 11/16/12. One for a well which was deemed improperly constructed and within 100 feet of the current source. The second was for no operations manual. The operator has submitted a wavier from construction standard application which is under review. The operator has agreed to a time line of September 2013 to have the operations manual completed. I have attached an email correspondence from Liza agreeing to the time line. The operator has asked for an extension due to major systems changes with the replacement of a storage tank that is causing PCE to enter the system. Jackson Creek is nearing completion of the state revolving loan process to replace the tank. ACTION NEEDED: Wait for wavier determination from the Springfield office hydro geologist. Review operations manual when completed in September. | ||
