Details: SUMMARY: No Backflow Prevention Annual Report DETAILS: Facility does not have a Backflow Prevention Annual Report and CCR. ACTION NEEDED: Facility to create and provide copies to the state a Backflow Prevention and CCR to tenants. Per email 6/25/13 from Debra she has sent the backflow report to the state and has handed out CCR to all of the tenants.
