| Details: | SUMMARY: follow up to sig. deficiences noted during survey DETAILS: Walt called earlier in the week to verify the paperwork he needed to submit to the state noted during the previous survey. On 7/23 a packet of paperwork was recived from Walt. The packet included: 2012 Consumer Confidence Report and certification form, 2012 Annual Summary Report, and cross connection ordinance. These were date stamped, copied, and sent to the Portland office for review. ACTION NEEDED: none at this time | ||
