| Details: | SUMMARY: Ecoli test result DETAILS: I was contacted by Melissa and she was concerned about a sample that was incorrectly mailed by the lab test America. The sample originally came in as an alert Ecoli positive. I had spoken to Ericka at test America and she confirmed it was a lab error and the results were not for this system. I spoke to her on 7/26 when the alert came in. I contacted Chuck Michael and discussed the issue. Chuck also discovered an issue with the April 2013 sample which did not appear to be correct. I called Melissa again and noted the circumstances related to the Ecoli positive sample taken in June. I documented the April and June 2013 circumstance in an email to Chuck Michael. I called Melissa and gave her an update. ACTION NEEDED: I advised Melissa to use only Alexin sample bottles. Alert removed. | ||
