| Details: | SUMMARY: Failed to take source sample 2012 DETAILS: I contacted Mike by phone and discussed the violation. He did not know an assessment sample is required every year. He will have the lab take a sample from the well right away. I asked him to also do a Public Notice and send in a copy to state ACTION NEEDED: Source sample from well taken right away or as soon as possible. Post public notice and send in a copy to the state. | ||
