| Details: | SUMMARY: Significant Deficiency (SD)survey follow up DETAILS: This was a follow up to The SD found during the survey from the deputy director of the facility, Karen. Lid to cistern found open and unlocked. Staff secured the lid and locked it. As part of the O&M plan, this lid and lick will be checked by staff every other month to be sure it is secure. No screen on cistern vent. A mesh screen was secured around the vent stack. No coliform sample plan. A coliform sampling plan has been written up and will be used in case of bad water sample. NOTE the double check valves after the well head do not have test coxs and cannot be tested. No further action required. ACTION NEEDED: Continue taking quarterly coliform water sample. | ||
