| Details: | SUMMARY: Regulatory Assistance DETAILS: System survey don Aug.15,2012.A survey letter was then sent to the system Aug.23,2012. A second letter sent described the requirements for pursuing a formal construction waive. A phone call was received on 12/10/12 from Vickie acknowledging receipt of the waive requirements letter and a discussion was held regarding corrective action. A follow up phone call was made to the system on 2/28/13. Vickie noted the system was awaiting a cost estimate from the city of Klamath Falls for a connection to city water. The system has failed to provide any evidence of corrective action since the waiver requirements letter was sent on Nov 14, 2012. A letter was sent today that cites a deadline of May 15, 2013 fro completing corrective action or providing evidence of approved scheduled corrective action ACTION NEEDED: Corrective action or evidence of approved scheduled corrective action by May 15, 2013 | ||
