| Details: | SUMMARY: Survey deficiency follow up DETAILS: A significant deficiency (SD) was cited 8/15/12 for not meeting setbacks. Four options for correction were provided in the cover letter. The owner, Anesti Audeh, requested an evaluation for a construction setback waiver. A letter describing waiver requirements was sent 11/14/12. Vickie Baker responded via telephone on 12/10/12 and stated the system would be investigating connection to city water or connecting to a second well approximately 100 feet to the south of the store. I followed up with Vickie on 2/6/13 and when stated the store was waiting on a cost estimate from the city for connection to the city system. She expects to have more information on 2/8/13 and will call back with an update. ACTION NEEDED: Connect to city water or complete plan review of second on site well and connect or continue to pursue formal construction waive requirements. | ||
