Details: SUMMARY: Survey deficiency follow up DETAILS: 6/15/13 survey showed two significant deficiencies. No gasket for reservoir hatch and no coliform sampling plan. Sampling plan was submitted Nov 2012 and a site visit 6/4/13 verified a rubber gasket on the reservoir hatch lid. ACTION NEEDED: none
