| Details: | SUMMARY: Significant deficiency follow up DETAILS: Follow up to significant deficiencies (SD) found during water survey. Concrete slap around well head is badly cracked and needs to be replaced or patched-- site visit and the concrete slab has been thoroughly caulked. Picture on file. Hose attached to raw water sample tap without an atmospheric vacuum breaker on the faucet-- hose not left attached to hose bib after use. Develop a coliform sampling plan-- the operator submitted a plan. Develop an O&M manual. The system has sent in details of what is now included in their O&M manual ACTION NEEDED: Continue taking routine coliform and nitrate samples. | ||
