Details: SUMMARY: Follow up for Significant deficiencies (SD) DETAILS: Survey on 5/14/13 identified two significant deficiencies. No Enabling authority (EA) for cross connection program and need for a new gasket on the reservoir hatch. Nate in office 6/28 and submitted signed EA and said a new gasket was installed when he was out having the EA signed. ACTION NEEDED: none
