Details: SUMMARY: PNC resolution, violations 1656212,1656218 and 1656220 and PN requirement DETAILS: System was in violation for not taking a source water sample in October of 2010-- a source water assessment sample was taken on June 11, 2013 and a triggered sample was taken on July 18th.
