| Details: | SUMMARY: Follow up for uncorrected deficiencies and pending NOV DETAILS: Recd call from Michelle Byrd re: pending NOV to be sent to this system for missing required documents from last survey (enabling authority, O & M, ERP, DRC). Circuit rider helped system in march and developed ERP, EA, O and M). These are in my file and will forward to state. Called Gary and discussed DRC form. I have sent him form several times in the past and he said he still has the form. Form must be completed and signed by owner Dave Hancock. I asked that the form be sumbitted by middle next week (sept 4, 2013). ACTION NEEDED: Complete and submit completed and signed DRC form by Wednesday 9.4.13. | ||
