| Details: | Contacted Tracy regarding water quality alert COLI17997. Facility had positive routine and repeat samples in previous month and is on temporary monthly samples. Facility was also asked to fill out level a 1 investigation form. Facility was unable to find this form and I provided the form to Tracy by email. The facility has been working with their inspector Phoebe Howe at Lane County and will follow up with the Level 1 investigation with her. A level two investigation has been triggered due to second round of temporary monthly testing positive. | ||
