| Details: | SUMMARY: Source assessment sample 2012 DETAILS: I contacted Mr. Ngo by phone and informed him of the violation and the requirement to take a source sample every year. He said the lab is suppose to do it and thats why he pays them. I let him know it still his responsibility to make sure it gets done and results are sent in on time. I also told him to post a public notice for minimum ob 7 days. I faxed a template to him and gave him the fax number for the state and informed him the completed form needs to be faxed in to the state. ACTION NEEDED: none | ||
