Details:	SUMMARY: New water system, requirements. DETAILS: System went through plan review process for re-activating an old well and then changed and got plan review conditional approval for a new well instead. The system serves water to a new Subway and Black Rock Coffee. They wanted to open ASAP and ended up getting an unofficial okay in an email from Jay M. to go ahead and open. I made a site visit today (10/3), and they were open. It turns out a chlorinator was installed and that was not included in the plan review. Chris said his contractor, Canden Development LLC, hired Mainline Pump & Irrigation LLC from Waldport to install the chlorinator. Jay contacted Canden to get them know they needed to go through plan review for the chlorinator. In addition, we told Adam from Black Rock they needed to take a distribution coliform sample prior to opening. Adam said they had taken one but it turns out it was a source sample. Adam saw someone checking the chlorine not taking a distribution sample. We told them they needed to get one taken today which they did and it came back ND. ACTION NEEDED: Make sure system completes plan review process for chlorinator.
