| Details: | SUMMARY: Required 30-day check-in following a survey with Significant Deficiencies DETAILS: Claude got the survey report, and took the one-time required arsenic test for the new well on 12/4/13, but has not yet received the result. Hes following up with the lab. He did not realize that he needed to do more for Plan Review of the new well, so he will review that part of the mailing as well as a plan-review related email Id sent. We also reviewed what he would put in the written Emergency Reponse Plan and Operations & Maintenance Manual. He says there are no seasonal or monthly maintenance tasks for his tiny system, so he will just include the relevant phone numbers in the documents - regulator, lab, emergency responders, pump maintenance person, etc. I asked him to put the sampling information there as well - the new coliform sampling plan, and indicate where he usually takes the annual nitrate sample. He knows he needs to inform me when these remaining significant deficiencies have been addressed. ACTION NEEDED: Await results of arsenic test, plan review materials and fee for the well, and his update that the written ERP and O&M manual requirements have been met for this public water system. | ||
