| Details: | SUMMARY: A source assessment sample collected on 12-18-13 tested positive for E coli. DETAILS: 12-20-13 --- John Benesch called and informed me that one of the five source confirmation samples collected on 12-19-13 tested E coli positive. I e-mailed him a Tier 1 boil water notice template with distribution instructions.12-23-13 --- John Benesch came by the office and we reviewed the information needed to try to determine whether or not the water system meets the 4-log disinfection standard and reviewed what is thought to be the well log. John will get back to me with more information ASAP.12-30-13 --- I forwarded system info provided by John Benesch to Scott Curry who determined that, providing a 1.0 ppm chlorine residual is maintained at the first user, the 4 log standard is met with the existing treatment/storage components-- therefore, the boil water notice can be lifted. I called John Benesch and relayed this to him.12-31-13 --- Source evaluation form sent to Shawn Stevenson - DWS Springfield. ACTION NEEDED: See above. | ||
