Details: SUMMARY: Significant deficiency corrected for well set-back hazard DETAILS: I received confirmation from Mark Hazelton that the inactive well located approximately 30 feet from the current active source has been formally abandoned. This was a significant deficiency noted in the 2013 water system survey conducted on April 8, 2013. He forwarded a PDF of the well log. ACTION NEEDED: Send well log the DWS Portland office
