Details: SUMMARY: Survey deficiency follow up DETAILS: Follow up for significant deficiencies noted during 1/23/14 survey. System new to inventory and missing ERP, CCR, O&M, EA, ASR and certified operator. Richard in office 3/14 and submitted ERP proof of completion (discussed erp and sent system PN templates as well as state ERP model). Richard attended SGWO course in Sunriver and submitted DRC form, Enabling Authority and 2013 ASR (although they are currently not sure how many DCVA are in) They will take an inventory and ensure all DCVAs are tested annually. System needs to replace two DBP samples with correct addresses and will sample this August. System also need CCR and will develop and submit prior to July 2014. ACTION NEEDED: Continue routine sampling for coliforms monthly, take 2nd round of Lead and Copper in March and take replacement DBP samples at correct locations. Submit CCR for 2013.
