| Details: | SUMMARY: follow up on significant deficiencies DETAILS: The 18-week deadline is approaching for resolving the 6 significant deficiencies identified in the last survey. Duane says he is nearly finished with the 2 paperwork deficiencies - creating an ERP and O&M manual. The deficiency for their monitoring not being current will take care of itself once they start submitting a coliform sample from all 4 quarters each year. We then discussed how he plans to address these: 1) the setback deficiency for their main well (83 ft to drainfield) - Duane will ask for a setback waiver-- 2) Wellhead not protected from flooding - Duane says he will describe how the seasonal wellhead in the subgrade vault is protected from flooding, including the reconstruction of the vault roof this summer (with a timetable for completion)-- 3) No raw water sample tap - Duane will describe how he collects samples that only represent Well 2s water using the tap down in the vault. ACTION NEEDED: Await written response from Duane Hand before May 30th deadline verifying that he has addressed each significant deficiency, and the date it was corrected, or providng a schedule for completion of any remaining signficant deficiencies. | ||
