| Details: | SUMMARY: 18-week deadline for correction of significant deficiencies DETAILS: Four significant deficiencies were identified in the last survey. Gary affirms today that two have been addressed: 1) He verified a week after the survey (so, Nov 2013) that there are no vents on top of the 2 indoor storage tanks, so no screening needed-- 2) He took the required Basics for SWS operators training class on 3/3/14, and is now sending in the application to our Portland staff. That leaves these deficiencies: 3) Lack of Emergency Response Plan, and 4) Lack of an O and M manual. We discussed again what these materials should address, and I contacted the Circuit Rider HBH, who will be able to assist them with these materials. I am approving a Corrective Action Plan that gives the school another 90 days to finish these tasks, due by July 21, 2014. ACTION NEEDED: Send HBH Engineering the request for assistance, the contact information for the operator and background information on the water system. Check back with operator in 30 days to make sure they made contact. Send operator a letter now giving the system another 90 days to complete the remaining significant deficiencies. | ||
