| Details: | SUMMARY: Survey follow up DETAILS: I contacted David because he had not contacted me within 30 days of the letter sent to him 10/21/13 regarding corrective actions from a water system survey performed on 10/10/13. Mr. Heath said he lost the letter I sent him and requested another copy. I will mail a copy of the letter to Mr. Heath on 1/24/14 ACTION NEEDED: Correct all deficiencies by 2/24/14 or be on an approved schedule of correction by that date. | ||
