Details:	Background/Problem Identification: Hanley Farm is a small groundwater system near Medford in Jackson County. OHA has requested that they fill out a Disinfection Verification Form. Tam Moore of Hanley Farm reached out to the circuit rider for assistance in filling out the form. Assistance Provided: The circuit rider filled out the Disinfection Verification Form on behalf of Hanley Farm Living History Museum. Follow-Up Actions Needed: The circuit rider is available to assist further if needed.
