| Details: | Background/Problem Identification: Z. Golik of OHA had requested assistance in regard to the Circuit Rider reaching out to Twilight View Estates to determine if their orthophosphate injection system was properly installed and running correctly. The Circuit Rider made repeated attempts to schedule a site visit. Several email addresses and phone numbers were used to no avail. Z. Golik of OHA was also trying to reach R. Boltz for some time, also with no result. Assistance Provided: The Circuit Rider was unable to make contact with the operator. Follow-Up Actions Needed: The circuit rider can provide follow-up assistance if needed once contact with the operator has been made. |
