| Details: | SUMMARY: New corrective action plan for remaining significant deficiencies DETAILS: While I was on leave, Erica had called to say that she could not get our PN template to open on her computer. The phone duty person suggested the i-writer, and that seemed to work fine. She has the Tier 2 Notice ready (for missing the 120-day post-survey deadline, which was June 12), and it will go out with the billings at the end of June. Only 1 significant deficiency was completed on schedule (the seals at wells were checked and fixed). 1 more was completed in June (Operator Protocols). Two remain (O and M Manual, Cross Connection testing), and we agreed on a Sept 30th deadline for the new Corrective Action Plan. ACTION NEEDED: Await copy of the Tier 2 Public Notice.
