| Details: | SUMMARY: Survey Deficiency Follow up DETAILS: Follow up for 3 significant deficiencies noted during 3/20/14 survey. -wellhead not sealed - Jerry sent me picture that opening was filled with silicone-No annual summary report - ASR was submitted and received by DWP-No current SGWO or DRC. Called Jerry 7/21/14 and he is scheduled to take SGWO source in Seaside in August and well submit copy along with DRC form. ACTION NEEDED: Submit SGWO certificate with DRC form as soon as completed. | ||
