Details:	SUMMARY: Violation correction903500419 and #903500410 DETAILS: The system was required to take source water assessment samples on a yearly basis due to the fact they had added a UV system without the benefit of plan review. The following violations occurred :violations #903500419 and #903500410. Since the WSS in April of 2014 the system was completely removed. At the time of the inspection the unit wasnt even plugged in. Please RTC the violations since they did not have an operation UV system and has since been completely removed ACTION NEEDED: Return to compliance
