| Details: | SUMMARY: significant deficiency correction from survey DETAILS: A water system survey was performed on 8/18/14. The follow deficiencies were noted and have been corrected: Management & Operations Violations - No operations and maintenance manual and Emergency response plan not completed-- Monitoring & Reporting Violations - No coliform sampling plan-- Source Deficiencies - Well Construction Deficiencies - Does not meet setbacks from hazards. ERP verification form was submitted and entered, coliform sampling plan on file, O&M is complete, well setback involves no more fertilizer application in the 100 ft setback area around the well.
