| Details: | SUMMARY: Survey Deficiency Follow up DETAILS: August 13, 2014 survey noted open electrical port on well head. Operator Joe submitted pictures and statement of correction on 11/5/14. Electrical port had been filled with silicone. *a survey follow up was also completed for 91022 (state park overnight) for a lock installed on reservoir hatch. These follow ups were submitted on same contact report but under only 91022. ACTION NEEDED: Continued monitoring and maintenance | ||
