Details: SUMMARY: System survey follow up DETAILS: The system operator sent the receipt and photo of the sample tap orientation change so water does not discharge on the well head. The operator also dropped off a completed ERP on 12/29/14. This resolves the deficiencies and violations from the 8/21/14 survey ACTION NEEDED: None
