Details: October 13, 2020 survey noted 3 deficiencies. No ASR,CCR,SGWO. Brian submitted 2019 CCR and certification 3/5/2021. He also stated that he submitted 2020 ASR electronically. System still shows SGWO, but Brian will be taking next available course and submitting form as new DRC.
