| Details: | SUMMARY: Regulatory Assistance DETAILS: Contacted Ronda Maze by phone on 3/04/15 to determine corrective action status of significant deficiencies. Ms. Maze was not aware of any construction work completed in the wellhouse and stated she would have the well contractor call with an update. Craig with Oregon Water Services called the same day and stated all repairs were completed in the well house. Information on the chlorinator and UV light would be sent to Casey Lyon to satisfy the plan review requirement. I visited the location on 3/09/15 and verified all repairs in the well house have been completed. Photos were obtained for record. Ronda Maze displayed a chlorine test kit and stated the store would test for residual chlorine 2X per week as required. ACTION NEEDED: None. All significant deficiencies are corrected. | ||
