| Details: | SUMMARY: Regulatory Assistance DETAILS: Barbara Silver was not available during follow up visit. Provided a copy of the sanitary survey letter that listed significant deficiencies to an employee. Ms. Silver emailed a photo on 3/10/15 that documented the addition of a hose bib just after the UV treatment. The owner also stated she would contact Jay MacPherson to begin plan review of new installed well. ACTION NEEDED: Complete plan review of new well and UV light. Complete one time arsenic test on new well. Follow up by 4/09/15. | ||
