I verified that he received the survey report, discussed the rule violations/deficiencies cited in the survey report, confirmed he understands his responsibility to submit written verification that the rule violations/deficiencies have been corrected and the dates the corrective actions were completed, or have an approved Corrective Action Plan in place by 4-6-2015.
