Details: SUMMARY: Water system survey corrections of significant deficiencies. DETAILS: 1. The wellhead is too close to potential hazards 2. Coliform sampling plan has been completed. 3. Operations and maintenance manual had not been completed.4. Emergency plan/vulnerability assessment had not been completed. ACTION NEEDED: 1. Potential hazards that were listed have been removed from the vicinity of the well .2. Coliform sampling plan has been updated and being followed.3. Operation and Maintenance manuals have been completed.4. Emergency plan/vulnerability assessment has been completed. Copy of proof of completion mailed to the State on 3/10/15.
