| Details: | I contacted Mr. Pate after not receiving response from the assigned operator. I outlined the process for accessing and submitting the ASR and CCR which was deficient in the 2020 survey. He informed me this would be a priority and asked for a month extension on this. I am setting the due date to 4/12/21. |
