| Details: | SUMMARY: Suvey follow up DETAILS: Contacted Brenda to find out if they had completed the CCR. She said Doug did finish it and sent it to all of the residents. I told her she needed to send a copy to the state along with the CCR certification form which I sent her via email. She sent the CCR and certification form back and I forwarded it to the state. All deficiencies corrected. ACTION NEEDED: none | ||
