Details: SUMMARY: Survey deficiency follow up DETAILS: Water system survey on July 1, 2014 showed no raw sample tap and taste/odor chlorine treatment did not go through plan review. Worked with system and Carrie Gentry to correct. Jim sent pictures of new sample tap prior to treatment on 3/23/15 and Carrie completed PR on 4/15/15. ACTION NEEDED: These deficiencies are considered corrected. Continue on-time sampling and take a and record chlorine levels after treatment twice weekly.
