Details:	SUMMARY: violations due to GWR DETAILS: System was required to take assessment samples from a new well with final approval on June 5, 2014. The samples were to be twelve months in length. System received two violations in regards to assessment sampling violation Numbers 900858520 and 900858519 twelve samples have been received all absent for TC- please RTC system. ACTION NEEDED: OHA to return system to compliance
