| Details: | SUMMARY: Survey deficiency follow up DETAILS: 4/15/15 survey showed no screen for well vent and missing coliform sampling plan and O & M. John mason followed up and Gene submitted photos of screens for well vents installed. Spoke with Gene 8/5/15 and I found a coliform sampling plan from 2013 for the system. Avion has O & M protocols for all their systems that they will apply to this system. Also, place all the water documents (erp, csp, survey) in one location to act as simple O & M.
