Details:	Alston Corners Assembly of God's Coliform Sampling Plan and Operation & Maintenance Manual forwarded to Columbia County Public Health was determined to have sections that were incomplete. The Coliform Sampling Plan did not list Repeat or Source sample locations in the event of a positive total coliform sample result. The O&M Manual's chemical sampling details section was missing. A water system operator that takes care of system operations was named, however, the specific system maintenance practices (maintaining gauges, valves, distribution system, collecting samples, etc.) on the O&M Manual such as did not list an operator name. I emailed Pastor Steve Berry to complete the Coliform Sampling Plan and Operation and Maintenance Manual, and to clarify some of those details for both documents. In the email, I also attached templates of the O&M Manual and Coliform Sampling Plan to complete and submit back to Columbia County and/or Oregon DWS.
