I verified he received the survey report and letter, discussed the rule violations/deficiencies cited in the survey report, confirmed he understands his responsibility to submit written verification that the rule violations /deficiencies have been corrected and the dates the corrective actions were completed, or have an approved corrective action plan in place by October 7, 2015 .
