| Details: | In response to the Deficiency Correction Overdue notice I received today, I forwarded the template for the Annual Summary Report (ASR) - Cross Connection for Deer Island Village MHP to Donna Holdridge and Jason Carman from RCAC who assists with the water system operations. In the email, I told Donna and Jason to complete the ASR document and forward it back to me, as I will forward it to Oregon DWS; they also may forward the completed ASR document to DWS-Cross Connection email, fax or mail address listed on the template. |
