| Details: | Last week I sent Rudy an e-mail reminder that the significant deficiency corrections for the 10/21/20 survey were due by 3/31/21. The deficiencies are for not submitting the annual consumer confidence report and the annual summary report. I texted Rudy this morning to remind him. Rudy requested an extension to have the items corrected. I granted a 30 day extension until 4/30/21. |
